When you have a suspected infection, two lab approaches dominate: PCR (molecular) testing and traditional culture. They sound interchangeable, but they work very differently — and modern PCR can now tell you not just what is causing an infection, but which antibiotics it is likely to resist, in about a day. Here is how they compare.
How does PCR testing work?
PCR (polymerase chain reaction) detects a pathogen's genetic material directly, amplifying tiny amounts of DNA or RNA until it can be identified with high precision. Because it reads genetic signatures rather than waiting for living organisms to grow, PCR is:
- Fast — results are often available the same or next day rather than days later.
- Highly sensitive — it can detect an infection even at very low levels.
- Specific — it pinpoints the exact pathogen, and can screen for several at once via a panel.
- Resistance-aware — a PCR resistance panel also detects antibiotic-resistance genes (such as those behind MRSA or ESBL-producing bacteria), so you learn which drugs are likely to fail before they are prescribed.
How does culture testing work?
A culture grows the organism in a controlled environment, then exposes it to antibiotics to see which ones work. It is thorough, but slow — typically 2–5 days — because it depends on live bacteria multiplying. For many common infections, that wait delays effective treatment while the patient is stuck on a best-guess antibiotic.
PCR vs. culture: which is better?
For most patients who need answers quickly, PCR wins on the two things that matter most — speed and resistance information:
- PCR identifies the pathogen and detects antibiotic-resistance genes directly from the sample, usually within a day, so therapy can be targeted from the start.
- Culture can still help for unusual organisms or when a full susceptibility profile is required — but it costs days you often do not have.
The bottom line: rapid PCR resistance testing gives you fast, actionable answers, while culture is the slower legacy fallback.
